Change of Contact Details

To provide the best services to you, we need to ensure we have your correct details. If you have recently changed your address, phone number, email address or name please let us know:

  • Online – complete the form below
  • In person –  if you are unable to update your details online you can visit the surgery with any supporting documentation and out reception team will be pleased to help.

Please note that if you change address, and no longer live within the practice boundary area, you will be asked to register with a GP surgery closer to your home.

Before submitting this form please check to make sure your new address fall within our practice boundary. If not you will need to register with a new surgery. Please let us know as soon as possible if you change your address, telephone number or name. It is important that we have up to date contact details in case we need to contact you.

Please note: If your name has changed due to Marriage or by Deed Poll, can you please provide us with a copy of the appropriate document (requirement of Department of Health).

Medical matters and official complaints cannot be dealt with via this form.
